Passport renewal application online (
DS-82 online) |
Fill out passport renewal application online. Click on "Create form" button and print the application. Be sure to sign and date your application. |

Letter of explanation |
The letter has to be addressed to US Department of State and explain in detail why you are applying for a second passport. |

Current valid passport |
You are required to present your primary
passport with your application. If your current passport is
within 1 year of expiration you will need to apply for passport renewal before applying for second passport. |

Letter of Authorization  |
Please print, sign, and date the letter. |
| 5 |
Two (2) passport photos |
Photos must be recent, identical, 2x2 inches, not from a vending machine. More on passport photo requirements  |

Travel itinerary(Rush Service only) |
- Copy of a plane ticket to a foreign
country, or - Copy of a travel itinerary with
the applicant's name showing dates and flight info, or
if neither is available and traveling for business -
A letter
from the employer |
