New passport application form . Pages 3 and 4 of this form must be presented
and signed in person at an authorized passport acceptance facility.
You must use this form IF:
1. You have never had a US passport
or
2. You are under age 16
or
3. Your most recent passport was issued more than 15 years ago
or
4. Your most recent passport was lost or stolen.
Please follow the instructions on pages 1 and 2. Print and complete page 3,
and print and read page 4 carefully before appearing with pages 3 and 4 at
the Acceptance Agent location.

Passport renewal application form. This form should be used for renewing a previously
issued US passport. Please use the following checklist to see if you are eligible
to apply with a DS-82:
1. You can submit your most recent passport
and
2. You were at least 16 years old when your most recent passport was issued
and
3. Your passport was issued within the last 15 years
and
4. You use the same name as on your most recent passport or since your
passport was issued, you have had your name changed by marriage or court order
and can submit proper documents to prove it.
If you cannot satisfy any one of the above requirements you cannot use the
Form DS-0082 and must apply for a new passport (form DS-0011).

Application form for passport amendment (name change) /add passport pages. This form should be used for
amending a previously issued US passport. This form should be used:
1. Whenever there is a change in the printed information (e.g., name-change) after your passport was printed
2. To extend the validity of certain previously limited passports
or
3. To add visa pages.

Lost or Stolen Passport application forms. This form should be submitted along with the DS-11
Application for Passport to apply for a new passport if your previous passport has
been lost or stolen.

Statement of Consent from a parent (for minors under the age of 14).
This form should be submitted, along with the DS-11 Application form for passport,
for minors under age 14 as evidence of parental permission if:
You are a non-applying parent or guardian consenting to passport issuance for your child, or
You are an applying parent or guardian, and the written consent of the non-applying parent or guardian cannot be obtained.
