Download and fill out this application form. Be sure to sign and date your application. If your passport was issued more than 1 year ago please follow the Passport renewal guidelines. |

Two (2) passport size photos |
Photos must be recent, identical, 2x2 inches, not from a vending machine. More on passport photo requirements  |

Proof of the name change |
A certified copy of either
- A marriage certificate, or
- A name change court decree

Letter of Authorization |
Print out 1 copy of this letter. Fill out all the necessary information and sign. |

Valid passport |
Make sure that the expiration date is more than 1 year away. If your passport expires within a year please apply for passport renewal |

Travel itinerary(Rush Service only) |
- Copy of a plane ticket to a foreign
country, or - Copy of a travel itinerary with
the applicant's name showing dates and flight info, or
if neither is available and traveling for business -
A letter
from the employer
